To become a new patient, please follow the steps below.

1. Complete our patient information form linked here: Patient Information Form

2. Next, complete our office’s intake/consent form linked here: Intake/Consent Form (If the patient is over the age of 18, they will need to complete this form. If an adult patient has a legal guardian or power of attorney, who makes medical decisions for them, that person will need to complete this form and provide documentation of guardianship or power of attorney).

Once both forms have been completed, a member of our staff will create a patient profile and reach out to schedule the initial intake/consultation appointment with one of our providers. Please note that a credit or debit card will need to be added to the patient’s file to secure the booking. Our office does not work with insurance and the cost of the initial intake/consultation appointment is $250.
